Lubbock’s Coordinated Care is a collaborative effort among providers committed to a community-wide coordination of housing and essential services through one shared community database.
Through a standardized intake and assessment process, called Coordinated Entry, people experiencing homelessness are connected to the right resources at the right time, based on their individual needs. This unified approach ensures access is fair, efficient, and dignified, helping those in crisis find stability more quickly through coordinated care and shared responsibility.
