How Coordinated Care Helps You:

How to Get Help
Through Lubbock's Coordinated Care:

  • The listed providers are designated coordinated care sites.

  • They use a shared community database to better understand and support your needs.

  • Staff are trained to help with prevention and diversion, aiming to keep you housed or find safe alternatives.

  • Some providers can do a formal housing assessment if you're experiencing homelessness.

  • This assessment may place you on the community’s housing priority list, which helps match people to appropriate housing.
